Farhat Medical Clinic is committed to the safety and well being of both patients and staff. In response to concerns surrounding COVID-19, We will keep our Clinic Lobby CLOSE due to  insufficient amount of PPE  available.  We will be available for our establish patients via TeleMedicine. We will be  available for certain procedures including injections by appointment only.  This is for protection of our patient and staff   We are asking that you contact the office prior to your appointment to receive the appropriate directions for TeleMedicine or in-person appointment for procedure including injection. 

Please continue to follow CDC recommendations to protect yourself and families from being infected with COVID-19.Even though some of the restrictions in our state have been lifted that does not mean the threat of infection is over. Avoid close contact with people who are sick. Wash your hands often with soap and water for at least 20 seconds. Avoid touching your eyes, nose, and mouth. Stay home when you are sick. Cover your cough or sneeze with a tissue, then throw the tissue in the trash. Clean and disinfect frequently touched objects and surfaces using a regular household cleaning spray or wipe. Practice social distancing. We want to caution our patients that even though businesses are reopening, the virus is still in our communities. You should wear a mask in public not to just protect yourself but others by doing so.

Instructions on Appointment for Procedure

Step 1: Please call to schedule if you need a procedure ( Injections, Trigger pionts injections, EKG). 

Step 2: You will be asked Patient Screening Questionnaire over the phone which you will be signing at your arrival on your appointment day. questions will consist of as follow:

  • Respiratory Symptoms(runny nose, sore throat, cough, shortness of breath) YES or NO
  • Fever of ≥ 100.4 degrees in last 72 hours YES or NO (can be measured or subjective)
  • New Loss of taste or smell? YES or NO
  • Nausea, Vomiting, or Diarrhea? YES or NO
  • Body aches or Pains? YES or NO
  • New onset of discolored toes (purplish or blue) that are swollen and painful to touch? YES or NO

Step 3: Once you get your appointment please arrive 5-10 minutes before your appointment time then call the office that you have arrived while staying in the car,  1 of our staff will let you know when we are ready to bring you into clinic, if you dont have a mask please ask for one, again this is for yours and our staff protection.

Step 2: Depending on provider, click the Blue Button 


Step 3: Wait in cue until your provider is available. If you step out of your cue, you may lose your place in line.
